STUDENT
REGISTRATION
To
make an appointment, call (732)525-5231.
You and your child must come to the appointment.
All registered students will be invited to a Kindergarten
Orientation in May or June to provide you with more information
about the district kindergarten program. |

*KINDERGARTEN ENTRANCE REQUIREMENTS*
Students entering Kindergarten MUST be five years old on
or before October 31.
ALL NEW students entering first grade
MUST be six years old on or before
October 31.
Children will not be registered for the Sayreville Public
Schools unless all of the following items are presented at
time of registration:
1. Child's official
birth certificate
2.Physician's certificate indicating that the child has
received the following immunizations as required
by the NJ
State Department of Health.
(a)Diphtheria, Pertussis and Tetanus(D.P.T.)-a minimum
of four(4) doses, one does having been given
on or after the fourth birthday or a total of five doses.
(b) Polio-a minimum of three (3) doses, one dose have been given on or after the fourth
birthday or a total of four doses.
(c) Measles- two (2) doses given at least one month
apart, both doses given after the first birthday.
(d) Rubella- one (1) dose after the first birthday.
(e) Mumps- one (1) dose on or after the first birthday.
(f) Hepatitus B- three (3) doses HepB vaccine or
laboratory evidence of immunity.
(g) Varciella one dose given on or after the first birthday or proof of the
disease immunity.
3. Certificate indicating that the child has had complete
physical examination
by his personal physician including a vision and
hearing screening,
within a year of September
entrance, e.g., the twelve months preceding start of school.
4.
Proof of residency as documented by a certificate of
occupancy, deed, mortgage agreement, borough tax bill, lease,
etc. as per N.J.A.C. 6A;28-2.5
5. It is recommended that the child receives a Mantoux Test
for Tuberculosis within six months of the start of the school
year unless she/he is required under current state
regulations. |

MEDICATION

Medication, including over-the-counter, may not be
administered without a written doctor's order and written
permission from the parent. Most medications can be given at home. Even
three times a day can be spaced out as...
1. Before school...
2. After school...
3. Before bed....
If your child does require medication during school hours, we
can work it out. Please call my office and I will send a
permission slip home.
Please remember ALL medication must be sent in the original,
properly labeled container. If another container is needed,
just tell the pharmacist it is for school. they will gladly
give you another container. |
ABSENCE

If your child is absent, please call
(732)525-5231
There is a recording available for early calls.
Please leave the
child's name, teacher and reason for absence.
When a child returns, the absence must be verified by
a note from the parent/guardian, even if you have called the
machine. A doctor's note is needed when a child has been out for
five consecutive days.
Homework may be provided after three days of absence. |
